This guide will help you complete the ADVOCO Application Form
& the check list for the Supporting Documents.

PERSONAL DETAILS OF APPLICANT 1 & 2

Surname:
as it appears on your photo ID.

First & Middle Names: your full name including any middle names this needs to match exactly to your photo ID ie. passport.

Date of Birth: as it appears on your photo ID.

Country of Birth: as it appears on your passport ID.

NZ Residency: do you have New Zealand residency?

Martial Status: what type of relationship are you in?

Smoker? if you have smoked cigarettes in the last 12 months either socially or occasionally. (this is for life insurance purposes.)

Age of Dependents: list your dependent children (under 18 years old) living with you.

Current Residential Address: current address you reside.
Postal
Address: if you wish your mail to go to another address eg a PO Box, insert here.

Time There: how long have you live there in years/months? If less than 3 years we require your previous address.

Previous address: insert previous address and how long did you live there in years/months?

Are you currently? Renting at your address above, Boarding Living in own your home or Other.

Home/Business/Mobile/Email: insert your contact details.

 

 

 


EMPLOYMENT

Main Source of Income: tick in box your main source of income. If Other; state type of income eg. NZ Super, Benefit

Occupation: what is your current job title; generally this can be found on your payslip, if PAYE.

Employer/Business Name: insert your employers company name and details. If self-employed; your company name etc.

Type of Business: what industry is your employer's business details. If self-employed; your industry etc.

How Long?: how long have your been with this employer in years / months. If self-employed, how long have you been self-employed?

Gross Income $: what is your annual gross income before tax.

If less than 3 years - prior to that: please insert your employers details and occupation. If self-employed your company name etc. How long were you with this company? If self-employed, how long were you self-employed?

Salary / Wage:
insert your annual gross income per year (before income tax). (We will calculate your monthly net income)

Business:insert your Self-Employed income Net Profit (before tax).

Benefits: insert your WINZ benefit as a net monthly amount. Include any Family Tax Credits as a net monthly amount.

Interest/Dividends: insert your gross weekly income from dividends from shares in public company (per year) and/or interest from term deposits and/or PIE investments (per year).

Other: Value of Fringe Benefits: list any non taxable allowance, car allowance, mobile allowance, accommodation allowance.

Rental (as below): see below insert address of rental property and exisitng your gross weekly rental income from rental property(ies)

Store Cards: list limit on credit card. Whether you use the credit card or not.

Child Support/Child Care: if you pay child support insert amount each month here. Or pay for child care insert amount each month here.

Rent to pay ($ per week): if you will continue to pay rent insert weekly amount here.

ASSETS:

Main Bank / branch: What bank do you hold your main accounts with.

Money @ Bank/Other: Cash (all bank accounts): list all cash either in bank account on term deposit, PIE accounts, deposits paid on property to Real Estate agent company.

Property - Family Home: full address of property you own and current rateable value or registered value.

Other Properties: full address of investment property and current rateable value or registered value either residential or commercial.

Public Shares:
amount of shares and name of public company and current value.

Vehicle 1: Year, make and model and current value.
Vehicle 2: Year, make and model and current value.
Vehicle 3: Year, make and model and current value.

Superannuation/KiwiSaver: list name of provider and amount of funds in KiwiSaver including government contributions. (SV) Surrender Value: insert SV amount here.

Life Insurance: insert the name and amount of life assured, if no (SV) Surrender Value leave blank. (Term Life cover has no surrender value.)

Business Value (net): list value of business including fixed assets. General rule of thumb is 2 x Net Profit plus fixed assets.

Boat/Caravan: insert value of your boat or caravan.

Other: list any other valuable asset eg intellectual property, loans from you to a third party, shares in private company.

Furniture & Personal Effects: insert value of your contents insurance.

You do not need to add the totals we will do this for you.

 

 

 

LIABILITIES:

Overdraft: name of bank, limit on overdraft and current amount owing.

Mortgages - residential: name of bank / lender and total of all mortgage balances including any Line of Credit limits whether used or not.

Mortgages - investment: name of bank / lender and total of all mortgage balances including any Line of Credit limits whether used or not.

Personal Loans:
insert name of bank/lender and include any loans owed to a bank or finance company or family members or friends whether making regular payments or not. eg. GE Money or finance company also include car loans eg. MTF and /or personal loans eg. ASB

Hire Purchase: include ALL loans, the name of the lender and total amount owing

Credit / Store / Charge Cards: include ALL, the card issuer, eg. Bank issued, Farmers card, Q Card, GE Credit Line, Warehouse card, include limit on the card and current balance whether you use or have used the card or not. If you pay in full each month tick box.

Student Loan: insert name and amount owing on student loan - if more than one loan list separately and amount owing.

Other: any other loans (not including bills); including repayment for outstanding debts to Dun and Bradstreet or Veda, Baycorp or utilities company or direct sales company. Or Business debt: include any business debt even if interest is being paid from the business profit and loss account or Commercial Loan: include the lender on any commercial property loans and amount owing.

You do not need to add the totals we will do this for you.

Please Note: It is a requirement of the application to disclose all assets and liabilities - failure to do so may result in a decline by a bank or lender. This includes any loan arrears or Property Law Act notice.

Supporting Documents for Loan Application

The Application Form requires the following Supporting Documents ( * mandatory requirement )

 Photo Identification * (copy of valid Passport or drivers licence or firearms licence)
A copy of valid NZ Passport or International Passport or NZ Driver Licence or NZ Firearms Licence. Passport is preferred.
   
The Anti-Money Laundering and Countering Financing of Terrorism Act 2009 (AML/CFT Act) requires photo ID to be verified and valid and mortgage documents must match valid photo ID.This needs to be certified by Mortgage Adviser/Bank/Lawyer before settlement.

 Proof of New Zealand Residency
(if not a NZ Citizen)
Current valid Residence Permit issued by New Zealand government.

 Income confirmation *
3 (three) consecutive payslip's showing name and dates of pay cycle or employer letter on company letterhead or IRD Certificate of Earnings from IRD. ^For self-employed YE 2013 & 2014 Full set of Financial Accounts. (see below)

Full 3 (three) months bank statements * of transactional account showing income crediting account.
Copies of original bank statements showing name of account and income crediting account or print outs from internet banking showing name of account and running balance. (Excel CSV. downloads are not acceptable) The bank statements must show income going into the account - if you transfer funds to another account then provide 3 months of that account. Also any accounts with Overdrafts.

Document showing evidence of your current residential address *.
Local authority rates notice or utility bill or bank statement or current vehicle registration or government agency statement.    

The Anti-Money Laundering and Countering Financing of Terrorism Act 2009 (AML/CFT Act) requires your address to be verified using acceptable documents this needs to be certified by Mortgage Adviser/Bank/Lawyer before settlement.


Evidence of Deposit * or Gift Certificate or KiwiSaver Statement.
Bank statement showing cash deposit or signed, dated and accepted Agreement of Sale and Purchase of Real Estate for sale existing property(ies) or Gift Certificate prepared by lawyer or Deed of Acknowledgement of Debt prepared by lawyer or KiwiSaver statement showing current balance.  

Evidence of 5% Saved Deposit
(first home buyers)
6 (six) months saving history showing regular deposits crediting account.

Signed and accepted Sale & Purchase Agreement (if available)
Signed and dated Agreement for Sale and Purchase of Real Estate.

Current Registered Valuation dated less that 3 months old
(if available)
Registered Valuation by valuer acceptable to the bank dated less than 3 months from application. If you do not have a current Valuation then we will arrange the Valuation through Property IQ, if required . This is a new requirement from all the banks.

Full 6 months mortgage repayment history (if refinancing)
Full 6 (six) months repayment history on all mortgage accounts including Line of Credit accounts - these mortgage/bank statements must show the running balance of your mortgage balance.  

Latest Council Rates notice
(if refinancing)
Local authority rates invoice showing rate payments.

Current house insurance policy showing lender as interested party
(if refinancing)
Current house insurance policy showing current lender as interested party.

 ^ Year End 2013 & Year End 2014 - Full Financial Accounts signed by Directors
(only if self employed)
Year End 2013 and 2014 Full Financial accounts prepared by Accountant showing Profit and Loss Account, Balance Sheet and Shareholder Equity and Deprecation Schedule - must be signed and dated by Directors of company.

Tenancy agreement or Rental assessment
(if property is tenanted or if purchasing a rental / investment property)
Current up to date Tenancy Agreement showing current rent and signed by tenant and landlord or rental assessment by Property Management company or Property Manager statements showing rent paid to account or rental assessment by Real Estate Agent showing market rent.

Trust Deed - (if the property is owned by a Trust or is to be purchased by a Trust)
Full copy of Trust Deed showing Trustees and Independent Trustees signed and dated or Trustee Certificate.
AML/CFT (Anti-Money Laundering and Countering Financing of Terrorism Act 2009) requirements will apply

Certificate of Incorporation - (if the property is owned by a company or is to be purchased by a company)
Copy of Certificate of Incorporation showing company status as registered.
